Individualized support delivered where the person already lives. From daily living skills to getting out into the community, home-based care builds independence in familiar surroundings.
Who it’s for
- Children, youths, and adults with cognitive disabilities or other support needs
- Families who want support at home rather than an out-of-home placement
- People working toward more independent living
How we provide support
We start with the person’s goals — learning to cook a meal, taking the bus, managing money, making friends — and build support around them. One-on-one support means the pace and approach fit the individual.
Sessions are scheduled flexibly around school, work, and family life. We use a trauma-informed, strengths-based approach that builds confidence as well as skills.
Supporting the whole family
When one family member needs extra support, the whole household feels it. We work alongside parents and caregivers, share strategies that work, and help families get the rest and resources they need.
