For professionals
Referring a client to Keyons Harmony
For social workers, case managers, hospital discharge planners, PDD and FSCD workers, and clinicians. Refer online or with one call — we take it from there with your client and their family.
How to refer
- Refer online (choose "Professional referral"), or call or email us with your name, role, and how to reach you.
- Tell us the kind of support needed, the community, and how soon. Online, you can also upload an assessment or funding letter.
- We contact your client and family to book a free consultation.
- With your client's consent, we keep you updated as care is set up.
Please don't email health information. The online referral is the secure way to send clinical details and documents: only our intake team can read it, and you can check its status there.
