Chronic Disease Monitoring

Regular vitals checks, wound care, and health monitoring by our registered nurses.

Seniors Home Care
A nurse checks an older woman’s blood pressure at her dining table.

Living with a long-term condition means watching for small changes before they become big problems. Our registered nurses help do that at home — so fewer things are missed, and fewer trips to emergency are needed.

Who it’s for

  • People living with diabetes, high blood pressure, heart conditions, or other chronic illnesses
  • Anyone with wounds that need regular dressing changes
  • Families who want reassurance that someone is keeping a clinical eye on things

How we provide care

Our nurses check vital signs, track symptoms and readings, care for wounds, and help the person understand their condition and treatment. Readings are recorded so trends are easy to spot.

When something changes, we don’t wait. We follow up with the client and family and, with their consent, keep the doctor and wider care team informed.

Building confidence

Good monitoring isn’t just numbers — it’s helping the person feel in control of their health. We explain what readings mean, encourage healthy routines, and answer questions in plain language.

How we care

How we treat every client

Whatever the service, the person comes first — not the task list.

  • Dignity first

We knock before we enter, ask before we help, and explain what we’re doing. Privacy and modesty are protected during personal care.

  • Their choices, their routine

The care plan starts with the person — how they like their morning, what they eat, what matters to them — and we follow it.

  • Trauma-informed

Safety, trust, and respect come first. We move at the person’s pace and never force care.

  • Culturally safe

We honour each person’s culture, language, faith, and traditions, and work with families and Indigenous elders where they want us to.

  • Families stay in the loop

You’re part of the care team. We share updates, listen to what you notice, and adjust the plan with you.

  • Nurses behind the team

Registered nurses provide clinical oversight, so care workers have backup for medication, wound care, and changes in health.

Do you replace my family doctor?

No. We work alongside your doctor and care team — tracking readings and symptoms between appointments, and sharing what we see so your doctor has a fuller picture.

What conditions can you help monitor?

Common ones include diabetes, high blood pressure, heart conditions, and wounds that need regular care. Tell us about your situation in the free consultation and we'll explain what we can do.
