Hospital Transition Support

Safe discharge home with care coordination, medication reconciliation, and follow-up.

Seniors Home Care
A caregiver steadies an older man with a walker as he comes through his front door.

Coming home from hospital should feel like a relief. But the first days and weeks are when confusion about medications, missed follow-ups, and falls happen most. We help bridge that gap so recovery continues safely at home.

Who it’s for

  • Seniors coming home after a hospital stay, surgery, or rehab
  • Families who can’t be there around the clock in the first weeks
  • Discharge planners looking for reliable support for a patient

How we provide care

We help plan the discharge and make sure the home is ready. Once home, we reconcile medications against the discharge instructions, help with personal care and mobility, and support follow-up appointments.

Early, frequent check-ins in the first days help catch problems — pain, infection, dizziness, confusion — before they turn into a return trip to hospital. Our registered nurses provide clinical oversight throughout.

Stepping back as recovery goes well

As strength returns, support can scale down. If longer-term help is needed, we’ll talk with you about next steps — including funding that may help.

How we care

How we treat every client

Whatever the service, the person comes first — not the task list.

  • Dignity first

We knock before we enter, ask before we help, and explain what we’re doing. Privacy and modesty are protected during personal care.

  • Their choices, their routine

The care plan starts with the person — how they like their morning, what they eat, what matters to them — and we follow it.

  • Trauma-informed

Safety, trust, and respect come first. We move at the person’s pace and never force care.

  • Culturally safe

We honour each person’s culture, language, faith, and traditions, and work with families and Indigenous elders where they want us to.

  • Families stay in the loop

You’re part of the care team. We share updates, listen to what you notice, and adjust the plan with you.

  • Nurses behind the team

Registered nurses provide clinical oversight, so care workers have backup for medication, wound care, and changes in health.

More questions and answers

When should we contact you — before or after discharge?

Before, if you can. The earlier we're involved, the smoother the move home. But call us whenever you need us — even after your loved one is already home.

Can you work with the hospital team?

Yes. With the client's consent, we coordinate with the discharge planner and care team so instructions, medications, and follow-up appointments are clear.
